Do I have to have the same certificate provider on both of the LPA forms. I am doing the second one several months later and will find it easier to use a different certificate provider

The answer is yes, it is perfectly OK to use a different certificate provider on each of the two LPAs. The critical thing is that the chosen certificate provider meets the necessary requirements!
